Green Mountain Senior Concierge helps older adults continue living safely and comfortably in their own homes.Monthly membership starts with four scheduled home visits. Each visit is shaped around the client's current priorities, appointments, and practical needs.Following every visit, a designated contact receives a detailed update which may include the assistance provided, upcoming appointments, items requiring follow-up, and relevant nonclinical observations.

Green Mountain Senior Concierge is owned and operated by Nolan Guiffre, LPN, an experienced Vermont-licensed practical nurse with more than 15 years of experience supporting older adults.Nolan has extensive experience working in long term care and assisted living, including supporting older adults living with dementia and other forms of memory loss. Nolan's nursing background brings patience, familiarity with healthcare systems, and the ability to recognize when a concern should be referred to the appropriate professional.Nolan lives in Colchester, VT with his wife and dog.
